Jeffrey H. Slade

Jeffrey Hollis Slade, 69, of Smithfield, Utah, graduated from this life on Tuesday, September 9, 2025, following a courageous battle with cancer. Jeff was born on September 23, 1955, in Mesa, Arizona, to William and Wylene Slade. He married Carol Diann Sharp on January 13, 1978.

He is survived by his wife, Carol; their children and spouses: Roger (Brenda) Slade, Amanda (Kurt) Adison, Rosie (Greg) Ashcroft, and Jason (Samantha) Slade; seven grandchildren: Brendan and Ava Slade, Isaac Adison, Gracie (Wyatt) Johnson, and Essie, Charlotte, and Parker Slade.

He was preceded in death by his parents William and Wylene Slade, and his sister Brenda Weaver.

A viewing will be held on Thursday, September 18, from 6-8 p.m. at Allen Mortuary of North Logan, located at 420 E 1800 N, North Logan, Utah, 84341.  The family extends heartfelt thanks to the nurses and doctors who cared for Jeff.
